As one of the youth case management providers for the south central Montana mental health region, CBS youth case management presently serves over 300 youth and their families.

Youth case managers work with the families to create caring and supportive relationships to motivate and challenge the youth.

The foundational goal of case management is to link a youth and family to mental health services and then empower them to follow the plan without the youth case manager.

Youth Case Management Provides:

  • Assessment of child and family needs
  • Planning for services with the youth and family, including a strengths-based service plan
  • Coordination and monitoring of treatment services
  • Linking the child and family to community resources and natural support resources
  • 24-hour crisis intervention
  • Mediation and advocacy on behalf of the youth and family
